  1. Sections I, IV, and VII on Pages 1 and 2 must be filled up by all participants.
  2. You are required to sign and date the form in these sections, and
  3. The signature has to be notarized.
Married TSP participants (based on their retirement coverage), will need to complete either 4. Section II or III. Married participants under FERS and those in the Uniformed Services Must have their spouse to sign the appropriate TSP form, and 5. The Spousal signature on the TSP form must also be notarized.

Form TSP 70 Checklist For Thrift Savings Plan Request for Full Withdrawal

If you are transferring your traditional (non-Roth) balance, you need to;
  1. Check the box in Section V, and
  2. Complete Page 4 with the help of your IRA trustee or plan administrator.
If you are transferring your TSP Roth balance, you need to;
  1. Check the box in Section V, and
  2. complete Page 5 with the help of your IRA trustee or plan administrator.
If you are transferring both traditional and Roth balances, you must
  1. check the box in Section V, and
  2. complete both Page 4 and Page 5.
